ARKANSAS GRADUATE NURSING EDUCATION STUDENT LOAN AND SCHOLARSHIP PROGRAM APPLICATIONS
Applications for the Fall 2012 and Spring 2013 semesters are available below. The deadline to your respective institution is June 18, 2012. For questions about the AGNSLS program contact Imogene Conway (pconway@uams.edu).

Students applying to U of A, UCA, or ASU are to mail their applications to their respective campus representatives (see applications below for further instructions).
NOTE: the following items must be included with all applications. Applications without these documents will not be processed. If you have applied before you are still required to submit all the attachments with each new funding cycle. Assuring your application is complete is the sole responsibility of the applicant.
SEND ALL SUPPORTING DOCUMENTATION WITH APPLICATION - DO NOT SEND SEPARATELY:
Letter of Acceptance: All students, including past recipients reapplying, must submit a copy of their letter of acceptance to an approved Arkansas graduate nursing program.
Official Transcript(s):
Graduate Transcript: If already enrolled in a graduate nursing program, submit a current unopened official transcript showing completion of grades for the semester prior to Spring 2012. If Spring 2012 is your first graduate school semester submit a copy of your undergraduate nursing transcript.
OR
Undergraduate Transcript: If a new graduate student beginning graduate course work in Spring 2012, you must submit an official transcript from only the program where you completed your bachelor's degree in nursing.
DO NOT SEND TRANSCRIPT DIRECTLY FROM INSTITUTION OF ENROLLMENT. Have transcript sent to you and include it with your application along with the other attachments.
Intent to Take Courses Statement Form (last page of application) signed by both you and your advisor (your program of study will not be accepted as a substitution).
Federal Tax Return: A copy of the first two pages from your 2011 federal tax return.
