SUMMER 2009 REGISTRATION

Make appointment to see your academic advisor relative to identifying appropriate courses for the SUMMER session. Ph.D. registration forms can be picked up in the College of Nursing Student Services Office, COPH 5th Fl, room #5234.
To complete the registration form:
- Fill in all blanks in the top section.
- Make a check mark to the right of the course numbers of those courses of which you are registering.
- Have academic advisor sign form and initial the courses you checked.
These forms must be turned in between April 20th and May 26th, 2009
Make sure you include:
- Multi-part registration form
- Copies of current proofs
Proof of the following documents is necessary for Summer 2009 pre-registration or registration. The following materials must be submitted with your registration materials:
- Current Arkansas nursing license (copy)
- Copy of current license for your state of residence.
- Immunizations: TB and Hepatitis B.
- If enrolling in clinical courses--evidence of current CPR certification (copy of certification)
- Notarized proof of 2000 hours of clinical employment as an RN
You can check your proof dates on line.
Proof of these documents may also be requested by the Graduate School or the University on the day of registration Tuesday, May 27, 2008. Please bring copies of these documents.
If all these materials are not on file and you have not paid, your registration process will not be completed and you are NOT registered.
Those students registering for Dissertation, Thesis, Nursing Research Project, Independent Study, Special Topics, or other variable hour courses should fill in the appropriate space the number of credits desired, and check mark the course to the right of the course number. Not all Faculty are available in the summer; please check with your advisor before registering for a research option course.
Please note the following: "A student must be enrolled in 1 credit of research project when the comprehensive examination is taken. (College of Nursing Catalog, p. 37.)
