CAR POOL REQUEST
The car pool request form
is due no later than May 22, 2009. This is for students living outside
of Pulaski county only.
ID PICTURES
All entering Little Rock campus students will have their ID pictures made on
May 20th following the HESI exam. Hope campus students will have their ID pictures made the
day of registration at 10 AM. This is the picture that will appear in the
yearbook and in the class composite that will be distributed to faculty
members.
This
does not obligate you to purchase a yearbook.
On the UAMS campus, the cost of the yearbook is contained in your
registration fee if you are a full-time student.
ATTN Hope
Juniors:
Registration for the Hope students is scheduled for-11 AM-12 Noon on Wed, May
27, 2009...but you need to be here by 10am to get your ID pictures made. Go to
Creative Services (G-level of Ed2) by 10am...Ed2/B104 to get them made.
TB SKIN
TESTS
TB skin
tests will be placed Wednesday, May 20, 2008 and read on Friday May 22, 2008.
A representative from Student-Employee Health will be present to place
your TB skin test. You must have this
done before you can complete the registration process. If you have had a skin test within the past
two months, you will need to bring a copy of those results to become
part of your file. The verification MUST be signed by the
health official who administered the test and include the date you had the test
done.
HEPATITIS
B VACCINE
All
students must have the Hepatitis B vaccine series. If you have already had the series of three (3) immunizations, then send proof to the
Registrar's Office by May 20, 2008. If
no proof is sent, then you will be immunized at Registration with the first of a
series of three vaccine injections. There is no charge for the injections. After the injection,
students will be checked for a reaction requiring a 20-minute waiting period.
IMMUNIZATIONS
Each
student must send immunization information with their
Physical Examination
Form to Student Employee Health (address on the top of the physical form)
verifying that he/she had received the immunization for Tetanus Diphtheria, and Measles, Mumps, Rubella and *Hepatitis B. Call Student Employee Health at (501)
686-6565 with any questions concerning immunizations. You will not be allowed to register if there is not a completed
physical form on file for you.
*(Student Employee Health and the
Registrar's Office will each need a copy of the Hepatitis B immunizations
and the TB skin test results.
Please provide two copies of these proofs--one for each of your
files.)
HOUSING
AND PERSONAL HEALTH INSURANCE

In
order to register for classes in the College of Nursing students will be required to show proof of current major medical coverage (e.g.,
insurance card or a policy showing current coverage) OR
purchase UAMS health insurance coverage; information concerning that coverage
can be found at
www.uams.edu/studentlife.
No one will be allowed to
register for classes, and therefore attend classes unless evidence of health
insurance coverage is provided.
Student Activities will forward information to you about the personal health insurance required for
all UAMS students. Health insurance
is to be paid by separate check at registration. If you are
already covered, you will have to provide a copy of your proof of insurance.

Costs
for meals in the UAMS cafeteria will be between $6 and $7 per meal. Students who display UAMS ID badge will
receive a 20% discount.
PARKING DECALS
Parking decals must be obtained from the UAMS Department of Public Safety
for parking in the gravel lots--first decal is free, any additional are at a
small cost . There is also free parking in the War Memorial Stadium parking lot
with free bus shuttle service from 6 AM - 7:30 PM weekdays only. Contact
UAMS Public Safety for information about evening parking lots and rates.

FINANCIAL AID
If there are any questions about financial aid, they should be

It will
be necessary for those students who are receiving aid to sign papers at
Program Orientation the afternoon after registration. A representative from the
Financial Aid office will be there with the necessary forms.
TUITION
Tuition is due in full on May 27, 2008.
LONG-TERM DISABILITY INSURANCE
Long-term disability insurance is required for all
students in the program. In order to
facilitate the coverage and provide a lower cost to students, a blanket
long-term disability policy will be utilized.
MALPRACTICE
INSURANCE
Malpractice
Insurance is required for all students in the program. In order to facilitate the coverage and
provide a lower cost to students a blanket malpractice policy will be
utilized.
CRIMINAL BACKGROUND CHECK
A Criminal Background Check is required for all
students in the program.
TECHNOLOGY FEE, LAB FEE, & MENTAL HEALTH FEE
The technology fee, lab
fee, and the
mental health fee are applied each semester.
CPR
CERTIFICATION
All
students in the College of Nursing are required to complete CPR
(cardiopulmonary resuscitation). We do
not accept ACLS certification. If CPR certification expires BEFORE MAY 2009, you must
be RECERTIFIED. Students will NOT be permitted to enter a clinical
course if your certification is not up to date. Below is the list of the ONLY CPR classes we will accept:
American Red Cross—Professional Rescuers Course—Good for one (1)
year
American Heart Association—Health Care Provider Course—Good for two
(2) years
BOOKS
AND
COURSE SYLLABI
Course
syllabi will be linked from the summer schedule.
The course syllabi will inform you of the exact titles and editions of
textbooks that are required. Textbooks
can be purchased on-line. You will need
to have your books and syllabi by Wednesday afternoon, May 27, 2009.
UNIFORM
AND SHOES
(to be updated) DO NOT PURCHASE
ANYTHING AT THIS TIME.
Students
will be required to have a uniform. You will need a lab coat immediately
and full uniforms by MONDAY, XXXXXXXXXX. White shoes free of any
other color or decoration should be purchased from the store of your choice.
