Advisor Forms (Deadline October 23, 2009):

Advising Forms Begin on October 12, 2009

Select the track you are on for the correct Advisor's Form:

Please read the following information very closely. 

To ensure that our RN students will not be required to come to Little Rock for registration, they must complete the entire early registration process, including payment of fees. 

Please be sure to read and follow each of the steps listed below:

Step 1 (October 1 – 12, 2009)

Meet with advisor via phone, email, face-to-face, to discuss which courses you will be taking for the semester.

Step 2 (October 12 – 23, 2009)

Complete Advisor’s Form on-line. 

 Advisor Forms (Deadline October 24, 2008):

Instructions for Advisor Forms

1)  Fill in all blanks in the top section.

2)  Place a check next to the course numbers of those courses which you plan to take.

3)  Hit the Submit button to send form. (NOTE: You must complete every required field.)

4) The RN Advisor Form will be submitted to the Advisors by the Registrar for Approval.

 Step 3 (October 12 - 23 2009)

Advisor will return the Advisors Form to the College of Nursing Registrar, Kristine Stump.  The student will be entered in courses; fees and tuition will be entered; registration packets will be mailed to the students.

the week of December 4, 2009.

§         Fee Sheet - This will show all courses in which student is enrolled and the total amount for tuition and fees.  Fees must be paid prior to the January 8, 2008 Registration Date.

§         Proof Sheet -  If any proofs expire prior to January 15, 2010 and the proof will be unavailable at registration, the student must attach a File Deficiency Contract.

Step 4 (December 4-31, 2009)

Complete and sign all paperwork included in the registration packet mailed to you, make sure all missing/expired proofs are included, and return packet to the College of Nursing Registrar:

Attach payment for all Tuition and Fees.

Students who do not return all required paperwork and pay fees by the deadline will be required to attend registration.
