RNs who plan to register for Spring 2008 classes:

RN Advising Form Due March 1

All documents that must be returned by January 8, 2008:

  • The enclosed fee sheet (Return ALL COPIES; a receipt will be mailed after registration.)
  • The updated/signed data form (enclosed)
  • Proof of current CPR, TB and Hepatitis injections
  • Current RN license
  • A check for fees

If  these documents and a check for tuition are not in our office by January 8, 2008, a $52.5 late fee will be assessed, and you will not be registered for classes.  (If your tuition is being paid by financial aid, please note it on the registration form.) A "date sheet" showing exactly what proofs we DO have for you is enclosed; please provide a copy of all that need to be updated.

All full-time students are required to have personal health insurance.

Your registration form and documents in paragraph one are due into our office by Tuesday, January 8, 2008.  Please affix the enclosed label to the envelope that you use to mail back registration materials to avoid lost registration forms and checks.

If you are eligible for the Employee/Spouse Discount, please obtain the form in your Human Resources office. Have the form signed by the appropriate persons and return it to Human Resources. They will process it and send it to the College of Nursing. It is your responsibility to verify that the discount form was sent to Kristine Stump by January 3, 2008. Please call her at 501-686-5113 to check on its receipt.  Any forms turned in after January 4, 2008, will not be processed until after classes begin. You will be charged full tuition, and the refund will not be processed until 3-4 weeks later.
