 Employee/Dependent Discounts (NOTE PROCEDURE)

The employee is to pick up the form from the Human Resources office.  Have the form signed by the appropriate supervisors and return it to Human Resources.  They will process it and then send it to the College of Nursing.  It is then the student’s responsibility to verify that the discount form was sent to Kristine Stump by Thursday, January 3rd .  Please call her at 501-686-5113 to check on its receipt.  Any forms received AFTER January 4 will not be processed until AFTER REGISTRATION.  You will be charged full tuition and the refund will not be processed until 3-4 weeks later.
