Health Assessment Theory and Practicum

Before you can enroll in these two courses

1. You must provide notarized proof of 2000 hours of work experience as an RN. This form may be obtained from this web site. (Experience Form)
   
   (Please note that an exact number of hours worked must be entered on this form.  We can accept that you have worked >2000 hours or 2000+ hours as an RN; however, we cannot accept that you have worked 40 hours a week for 2 years.)
   
2. You must have permission from your advisor (his/her initials on your registration form). 
   
3. A lab fee of $262.50 is charged for the practicum course and must be paid at registration.
