Required Proofs for Registration

Before you will be allowed to register, the following proofs must be current: 

PhD Students:

• RN license 
• TB skin test
• Series of three hepatitis B injections (or test showing antibodies are present)
• SMII (Student Malpractice Insurance Information Form--available on the web site)
• CPR
• Background Check form completed if you did not register last semester.

MNSc Students:

• RN license 
• TB skin test
• Series of three hepatitis B injections (or test showing antibodies are present)
• SMII (Student Malpractice Insurance Information Form--available on the web site)
• Background Check form completed if you did not register last semester.

Before you enroll in a practicum course, you must also have:

• Notarized proof of 2000 hours of employment (as an RN in clinical practice)
• CPR (ACLS is NOT acceptable)
              American Heart Association--Health Care Provider OR
              American Red Cross--Professional Rescuers

Please provide copies of any required proofs with your registration form. 

If the required proofs are not current until June 19, 2007, (two weeks past the day of registration), you will not be allowed to complete the registration process or attend classes.
