Make appointment to see academic advisor relative to identifying appropriate courses for the Fall session. If you are not sure who your advisor is, please contact Kristine Stump at (501) 686-5113.

Return all required document and forms to the College of Nursing Registrar, Kristin Stump,  4301 W Markham #529, Little Rock, AR  72205 by Friday, November 13, 2007.  This includes:

ü      Multi-part registration form

To complete the registration form:

1.       Fill in all blanks in the top section.

2.       Make a mark to the right of the course numbers of those courses for which you are registering and have your advisor initial the mark.

3.       Have academic advisor sign form.

ü      Copies of current or missing/expired proofs.

• If any proofs expire prior to August 30, 2008 and the proof will be unavailable at registration, August 14, 2008, the student must attach a File Deficiency Contract.

ü      Data Sheet - Demographic Information

ü      Health Insurance Certification (All Graduate School Students)

The following forms will need to be returned for all New Students

ü      Criminal Background Check

ü      Confidentiality Agreement (New Students Only)  

ü      Catalog/Handbook Statement

ü      HIPAA

ü      IT Confidentiality Agreement

If all of required materials are not received,

your registration process will not be completed.

Other Information:

 ¡         You are not registered for classes until process is completed and fees paid to the Graduate School.

¡         Those students registering for Nursing Research Project, Independent Study, or other variable hour courses should fill in the appropriate space with the number of hours you want to take, and mark the course to the right of the course number.

¡         Students registering for  Research Project:
(Reminder: At the time you are registering for your initial dissertation/thesis credits
 you must submit a Request for approval of Thesis Advisor and Committee Members form.)

¡         Please note the following:

¡         "A student must be enrolled in 1 credit of research project in order to complete comprehensive examination." (College of Nursing Catalog, p. 36.)
